Investigation Summary
At 10:30 p.m. on July 31, 2018, Employee #1 turned on the conveyor belt to clear out backlogged empty cardboard boxes. The employee's arm was caught and pulled in between the running conveyor belt drum roller assembly and the conveyor motor enclosure of the trash line conveyor system. The employee sustained multiple fractures, cuts and bruises to the arm and hand. The employee was hospitalized.
