Investigation Summary
At 4:52 p.m. on July 29, 2018, Employee #1 was removing relays from demo panels. The employee attempted to cut a lead wire, connected to a bus bar. The wiring was energized with DC voltage of 240 volts, that shocked the employee after the cutting wire. He sustained second and third degree burns on his arms, hands, and fingers. The electrical equipment/wiring was improperly locked out. Employee #1 was hospitalized for two or more days.
