Investigation Summary
At 9:30 a.m. on August 8, 2018, an employee climbed over a balcony wall from a scaffold when the scaffold back rail the employee was using for support broke free. The employee lost his balance and fell striking the mid-rail of the scaffold, which also broke free and allowed the employee to fall forty feet. The employee was hospitalized with a fractured heel, multiple pelvic and vertebrae fractures.
