Investigation Summary
At 1:30 p.m. on July 19, 2018, an employee was standing on an elevated work platform when it collapsed. The employee fell and struck the forks on a forklift and suffered a concussion, several broken ribs, a hand injury as well as laceration to his head. The employee was hospitalized.
