Investigation Summary
At 3:30 p.m. on August 2, 2018, three employees were replacing a water pumps that was part of Paper Machine #2. The employees replaced the pump and were aligning the motor when the Shift Supervisor started the machinery before the repairs were completed. Employee #1 amputated several fingers and all three of the employees sustained puncture wounds and lacerations to their heads, torsos, arms, and hands. All three employees were hospsitalized.
