Investigation Summary
At 8:15 a.m. on July 25, 2018, an employee was removing material that was built up from a conveyor's tray sensor on the TiAl weight blend system. The employee reached into the machinery when it started and caught his upper left arm being struck by the conveyor's reciprocating motion, fracturing it. The employee was hospitalized.
