At 9:00 a.m. on May 21, 2018, an employee was using a 1956 Monarch Machine Tool Co Lathe Machine Number 0253, located in the tool and die room. The employee had completed setting up the lathe and starting to turn down a rubber slitter rollers on a shop made fixture. As the lathe came up to speed that was set for approximately 685 RPM, the employee was struck in the face by the steel fixture cap when it spun off. The employee was hospitalized.
