Investigation Summary
At 1:25 p.m. on April 12, 2018, an employee was operating and removing caps from a machine. The employee reached into the knife area of the machine to remove caps which were stuck in this area. The area was not properly machine guarded. The employee's right thumb came in contact with the knives resulting in an amputation and hospitalization for over 24 hours.
