Investigation Summary
At 1:45 a.m. on July 10, 2018, an employee was adjusting the chuck and held a roll in position to close and secure it on the machine. The employee released the sidewall carriage chuck to decrease tension and used his left hand to manually operate the lever while holding the core with his right hand. The employee caught his right thumb between the roll core and the sidewall carriage chuck, fracturing and amputating his thumb. The employee was hospitalized.
