Investigation Summary
At 8:30 a.m. on January 29, 2018, Employee #1 was operating a 100-ton press. The press ejects wax through a slot. As Employee #1 placed his hand in front of the slot that ejects the wax to clean it off, the press came down and amputated his finger. He was hospitalized for treatment.
