Investigation Summary
At 11:45 on July 2, 2018, Employee #1 topped out a tree and identified another tree that needed trimming. He attempted to anchor in the next tree by scaling upward and reached toward the next tree's attachment location. While moving to relocate the climbing line (life line) into an adjacent tree, the employee stood up. The climbing line that was moved from the tree had not yet been secured to the target tree. His buck strap (safety strap) slid over a stub/top of the tree and the employee fell 40 feet to the ground below. Employee #1 sustained 3 broken ribs and bruises on his right side. He was hospitalized for his injuries.
