Investigation Summary
At 2:00 p.m. on July 5, 2018, Employee #1, an administrative worker, was gathering documents when the paper fell to the ground and the employee leaned against the machine to lift herself up. The machine roller was still operating at the time and in motion. The rollers were unguarded and the employee's arm was caught in the roller. He sustained a fractured left arm and was hospitalized.
