Investigation Summary
At 1:30 a.m. on June 26, 2018, Employee #1 and a coworker were directed by a Driller to get a casing basket using a Case forklift, Model 588H. The casing basket was 86 by 45 by 11, and held equipment needed for drilling. The previous crew had moved the casing basket under the casing pipes that were stored on top of two pipe racks, elevating the casing pipes approximately six feet off of the ground. About 20 joints of 2.875 tubing was then delivered and placed in front of, but parallel to the casing pipes on two boards on the ground. The tubing was blocking the access to the casing basket. The coworker operated the forklift and drove over the tubing, placing the forks under the casing basket. Employee #1 stood to his left, approximately five feet away from the forklift and three feet back from the tubing on the ground. The employee told the coworker that he needed to get the forks further under the basket before lifting it. As the coworker attempted to move the forklift forward, the tubing under the tire rolled back and over Employee #1s lower leg. His leg was trapped under the tubing and he sustained a fracture to his left leg that required surgery and hospitalization for two days.
