Investigation Summary
At 1:30 p.m. on June 21, 2018, an employee was working in a warehouse that distributes food restaurants and supermarkets. The employee was stacking boxes on the third level of the facility, when he slipped and fell approximately 20-25 feet. He struck a concrete floor and sustained injuries to his head, right shoulder, and back, which he was hospitalized and received treatment. The incident investigation identified that there was no guardrail in place from the side the employee fell. Instead, there was a static lifeline, but the employee was not provided a harness and lanyard.
