Investigation Summary
At 12:10 p.m. on April 30, 2018, an employee was closing the seam of the tarp while standing on the ladder. The employee kicked the tarp loose when the wind blew it into the ladder and the employee fell off of the ladder. The employee dislocated his shoulder and lacerated his head.
