Investigation Summary
At 9:00 a.m. on June 21, 2018, Employee #1 retrieved a pallet of equipment using a Crown electric riding pallet jack, Model number PE4500-60 and Serial number 6A276483. The employee backed the Crown electric riding pallet jack into the rear of a Caterpillar sit down fork lift, Model number C5000, and Serial number AT9005782. When the employee struck the forklift, his right hand was caught between the controls of the pallet jack and the propane tank on the forklift. Employee #1 received a laceration to the back of his right hand, and the bones of his palm were fractured, resulting in hospitalization and surgery.
