Investigation Summary
At 8:30 p.m. on June 20, 2018, Employee #1, a maintenance mechanic, was attempting to clear a fruit pit from the feed line pump for the Orics fruit cup machine. In order to perform this function, he switched off the pump and, using his hand, pulled the pump drive belt in the reverse direction. He then turned the pump switch back on in order to see if this was successful. He repeated this process until the pit was cleared. At the end of the last attempt to clear the pump, the pump started, catching his fingers between the motor belt drive and the sheave. The employee sustained an amputation to the tip of his middle finger of his left hand.
