Investigation Summary
At 4:30 a.m. on June 18, 2018, Employee #1 was driving a standing powered industrial truck (PIT). The fork of another PIT slipped under the standing PIT. The employee attempted to get the fork unstuck from the standing PIT. The fork released and the weight caused the load to rebound and pinch the employee's foot. He sustained a foot contusion and was hospitalized.
