Investigation Summary
At 9:48 p.m. on June 14, 2018, an employee was dumping bags of mortar mix into the M-Tec MR 90 V mixer to rework the material. The employee heard clinking noises and knew he needed to remove the rocks to prevent clogging down below. The employee turned the mixer off at the computer and then turned off the disconnect lever and returned to open the door at the mixer. The employee reached into the mixer, about elbow deep, when his left arm was caught by the rotating arm. He was taken to the hospital by EMS and suffered crushing and degloving injuries to the arm below the elbow. The interlock to prevent the door from opening prior to the arm coming to a complete stop was not functioning, allowing the employee to be exposed to the moving arm.
