Investigation Summary
At 7:30 a.m. on June 11, 2018, an employee removed the chain and sprocket guard from a flour tortilla machine press to replace the Teflon that was on the shaft. The employee caught his pinky between the chain and sprocket when the machine activated and fractured his finger. The employee was hospitalized.
