Investigation Summary
At 9:00 a.m. on June 6, 2018, Employee #1 was working on a machine, polishing a part. The part that the employee was working on, flew off of the machine and hit the employee in the head and shoulder. He suffered a broken left shoulder and lacerations to his left eyebrow, that required stiches. Employee #1 was hospitalized for treatment.
