Investigation Summary
At 8:00 a.m. on April 20, 2018, an employee was operating the SMETCO pallet sorting system and was inspecting pallets when the stack straightener stopped working. The employee placed his body between the stacked pallets and straightener, triggered the photo eye, and the walls started to move, crushing the employee.
