Investigation Summary
At 4:00 p.m. on May 22, 2018, an employee was walking around a waste truck and stepped on a piece of scrap lumber with a protruding nail. The nail punctured the employee's shoe and the sole of his right foot near the large toe. Several days later the wound had not healed and began to drain. The employee went to the Emergency Department at Riverside Community Hospital where they amputated the large toe and was admitted for eight days.
