Investigation Summary
At 10:15 p.m. on May 16, 2018, an employee was moving an electric pallet jack and pressed the gas pedal rather than the brake. The employee tried to jump off of the machine and caught his foot between the jack and a storage rack, fracturing his ankle. The employee was hospitalized.
