Investigation Summary
At 2:30 p.m. on May 23, 2018, an employee from a staffing agency was working on a forklift in a warehouse when he came across an electric pallet jack that obstructed his intended path. The employee was not authorized to operate the electric jack but attempted to move it out of the way. When the employee engaged the throttle, the electric jack moved in the opposite direction intended. The jack knocked down the employee and ran over the left foot, resulting in a laceration.
