Investigation Summary
At 3:45 p.m. on May 25, 2018, an employee was standing on an energized 6.9Kv transformer (XFMR #1438) located in a Buried Underground Residential Distribution (BURD) structure. The employee attempted to move the old concentric cables (ground wires) while standing on the XFMR when his right wrist contacted the secondary wires and his right thigh touched a metal object. The employee received an electrical shock and suffered second and third degree burn injuries to his right wrist and right thigh. The employee was transported by ambulance to Arrowhead Regional Medical Center before being transferred to Grossman Burn Center.
