Investigation Summary
At 1:33 p.m. on May 23, 2018, a driver for a recycling company was attempting to cover the top of a roll-off container full of metal with a tarp. The driver was walking on the top edge of the container while pulling the tarp. The top edge of the container was approximately 5 inches wide and 8 feet from the ground. While pulling the tarp, the driver tripped over a piece of metal that was sticking out over the edge. The driver fell to the ground, resulting in two fractures to the right leg.
