Investigation Summary
At 9:45 a.m. on May 17, 2018, an employee was working at Autoline 4 Domestic Packaging and was adjusting the poly bagging seal operation due to a jam. The employee was pulling a small section of polybag through the bag sleeve when he jogged the start button. The employee amputated his left middle finger when it was caught in the nip point between the two heated blades. The employee was hospitalized.
