Investigation Summary
At 8:00 a.m. on May 17, 2018, an employee was installing a nylon strap as a choker to a 5 inch steel pipe. A Terex crane was used in a attempt to lift an 18 foot long pipe. The boom of the crane was in proximity of the over head transmission lines. When the employee grabbed the ball/hook attached to the boom of the crane, it contacted 69 KV and 13.8 KV overhead power lines, resulting in shocks and burns.
