Investigation Summary
At 11:26 a.m. on April 9, 2018, Employee #1 was operating/overseeing the Continuous Bin Tipper machine. He was using a grabber to remove the leaves that were being transported with the avocados as they rolled on the conveyor. The employee removed the gate panel by loosening the top and bottom bolt. As the conveyor moved, he was trying to also get the bins unstuck. The bottom bin had a broken leg, causing it to get stuck at an angled position on the conveyors edge. When he tried to push it, the top bin was still touching the bottom bin, and it lost its hold from the robotic arm, falling and crushing his finger. Employee #1 lost the tip of his index finger, and was taken to a nearby hospital, where surgery was performed.
