Investigation Summary
At 2:45 p.m. on May 2, 2018, an employee was injured while switching products in the production area of a wholesale bakery. Employee #1 was standing on a rolling ladder, scrapping the batter off the Hinds Bock cake batter dispenser, and putting warm water into the hopper to rinse it. Employee #2 was at ground level with a pan under the machine to catch the water. The employees did not disconnect the product valve air cylinder and did not disconnect the airline that powers the machine. The machine was engaged and the product valve turned causing the finger of Employee #2 to get caught on one of the openings of the valve. When the piston closed, it crushed her right finger causing a partial amputation.
