Investigation Summary
At 8:30 a.m. on May 9, 2018, an employee was driving a forklift in aisle #26 and attempted to make a left hand turn onto speed aisle #2. The employee lost control of the forklift in the middle of his turn and slid into the storage rack. The employee's foot was pinned between the truck and storage rack and was later amputated. The employee was hospitalized.
