Investigation Summary
At 4:00 p.m. on April 9, 2018, Employee #1 and a coworker were cutting holes into a 0.5 inch thick stainless steel metal sheet, using a plasma cutter, in an outside work area. The employee was holding a manual compass that was used as a guide to cut the circular shapes onto the metal sheet, while the coworker was operating and cutting with the plasma torch. The employees shirt caught on fire from the sparks generated by the plasma cutting process, resulting in severe burns to his left torso, arm, and hands. He was wearing a welding helmet, leather gloves, 100% cotton shirt, pants, and leather boots. Employee #1 was hospitalized for his injuries.
