Investigation Summary
At 12:30 p.m. on April 10, 2018, Employee #1 was helping smooth out material on the press that had become crimped. The press operator activated the press, causing the press to engage and the die blocks to come down on the material before the employee could remove his hand. The guide bars on the die plates of the press caused a fingertip amputation.
