Investigation Summary
At 10:21 a.m. on April 11, 2018, Employee #1 was driving a John Deer 6300 tractor with a mechanical transplanting machine attachment. There are four stations where the employees sit in front of a sorter that rotates and funnels the plants into the soil. The movement of the tractor in turn, allows the panels containing the plants to rotate in front of the employees, and also dispenses the plants into the soil. The chain and sprockets are located behind each of the seats and they are unguarded. Employee #1 attempted to make an adjustment to the transplanter while it was still in motion, and his right hand became caught between the chain and chain sprocket. The employee suffered compound fractures to his wrist and arm. He was taken by ambulance and admitted to a nearby hospital, where he underwent reconstructive surgery to his wrist and remained at the hospital for nine days.
