Investigation Summary
At 10:15 a.m. on April 7, 2018, Employee #1 and a Coworker, employed by a mechanical contractor, were working at a food manufacturing facility. They were installing an overhead drip pan measuring 88 inches by 40 inches. Both were working from ladders. The drip pan shifted, swung down, and struck the ladders. Employee #1, who was on the first or second rung of a 4-foot platform stepladder, fell to the tile floor, a fall height of approximately 2 feet. Emergency services were called, and Employee #1 was transported to the hospital. He was admitted and treated for multiple compound fractures of his left upper arm (the humerus).
