Investigation Summary
At 6:30 p.m. on March 23, 2018, an employee was assisting a coworker with cleari ng a jam when the coworker reversed the blade direction on the airlock. The emp loyee had placed his hand into the output feeder and was struck by the blade. T he employee amputated three fingers from his left hand and was transported to Ha rbor UCLA for medical treatment. The employee was hospitalized.
