Investigation Summary
At 8:00 p.m. on March 22, 2018, an employee was inspecting the in-feed conveyor and cleaning broken glass bottle pieces from around the machine. The employee reached into the conveyor to remove a piece of glass and caught her right index finger in a nip point between the running belt and pulley, amputating it. The employee was transported to OSF Saint Anthonys Emergency Room by her manager and was hospitalized.
