Investigation Summary
At 1:30 p.m. on March 13, 2018, Employee #1, employed by a construction company, was working at a multiemployer construction project, a new commercial building. He was using a shovel to remove mud and debris from a trench when the trench collapsed. The collapse caused a mini-excavator to fall and strike Employee #1, who was caught between the wall of the excavation and a rebar cage attached to the excavator. Emergency services were called, and Employee #1 was transported to the hospital. He was admitted and treated for punctured blood vessels in his abdominal area.
