Investigation Summary
At 6:00 p.m. on March 14, 2018, Employee #1 was drilling a roll pin out of the shaft for the front wheel of an electrically powered pallet jack. He was using a hand drill with a 0.125 inch drill bit. The tip of the drill bit shattered, and a piece of the bit found its way past his safety glasses and injured his eye. The employee was hospitalized for two or more days.
