Investigation Summary
At 8:20 a.m. on March 6, 2018, an employee was setting up a wire draw machine, located at Draw Line #2. The wire draw machine was not locked out prior to the set-up activities. As the employee was jogging the machine by pressing the button on the control panel with her left hand, she was touching the wire that was being wound onto the main hub with her right hand. The employee's right hand was in an immediate proximity to the nip point created between the wire and the main hub, which was under an approximate 2,500 pounds of pressure. As a result, the employee's right index finger became caught in that nip point, partially amputating it.
