Investigation Summary
At 12:00 p.m. on March 7, 2018, Employee #1, employed by a mechanical systems contractor, was working at a multiemployer construction project, a six-story multifamily residential building. He was installing ducts for the heating/ventilation/air conditioning system. A fire broke out in the structure, and the fire department responded.
