Investigation Summary
At 4:00 p.m. on February 16, 2018, an employee was using a Big Joe Joey Model #J1-162 forklift, serial #F24120084, while putting a customer's order together and tried to slow down to make a turn when coming up to a storage rack. The employee was not able to slow the forklift to make the turn and crushed his right hand between the forklift and storage rack. The employee was hospitalized.
