Investigation Summary
At 5:00 a.m. on January 22, 2018, an employee was operating the tubing cutter when he reached into the loading area of the machine while it was powered on and caught his left hand in the chain and sprocket. The employee partially amputated his left hands middle finger and was transported to Sharp Grassmont Hospital, where he was admitted overnight.
