Investigation Summary
At 10:30 a.m. on January 27, 2018, an employee took his invoice to the receiving clerk in a loading dock through the side entrance ramp and then proceeded to retrieve a forklift bailing wire from his truck. The employee then drove a forklift to the receiving dock area and climbed on its forks to access the loading dock area. After handing over the bailing wire to the receiving desk clerk, he stepped back onto the forks f the forklift he had been operating. As he climbed down from the forklift forks, he fell approximately 4 feet and sustained a fracture to his left wrist and hip. He was hospitalized for treatment.
