Investigation Summary
At 7:00 a.m. on February 27, 2018, Employee #1 and #2, two maintenance workers, were performing maintenance on a monorail conveyor system, in order to bring the conveyor's chain closer together. The monorail conveyor system is approximately 15 feet above ground level. As the employees began to bring the conveyor's chain closer together, Employee #1 got on top of the conveyor systems to work with the belt and pulley, while Employee #2 worked underneath the conveyor. In the process of manually bringing the chain together, Employee #1's finger's became caught in between the belt and pulley, resulting in a fingertip amputation. He was hospitalized for his injury.
