Investigation Summary
At 6:05 a.m. on March 2, 2018, an employee was cleaning and making repairs to a water table that was used with the plasma arc cutter. The employee had drained the water from the table prior to making repairs and a buildup of hydrogen gas collected in the bladder of the water table. The employee climbed on top of the water table to begin working and the following day, the table exploded and he suffered multiple injuries including amputation of both legs, head trauma, fractured arms and ribs, and thermal burns. The employee was hospitalized.
