Investigation Summary
At 10:45 p.m. on February 24, 2018, Employee #1 was removing wire from the lifting mechanism. As the wire was being cut, the poles shifted and one of the poles fell on the employee's foot. The pole struck his foot just beyond the steel toe on his boot, resulting in the amputation of two toes. He was hospitalized for treatment.
