Investigation Summary
At 12:15 p.m. on February 22, 2018, an employee, who was a contractor, was working on a new residential construction site conducting final interior home cleaning on the homes just before they were turned over to the new owners. The employee set up an extension ladder against the wall to clean a second story window on the inside of the house just above the front door. There was cardboard used on the tile floor to protect the flooring and the ladder was set up on top of the cardboard. The bottom of the window measured just under 14 feet. The employee ascended up towards the top of the ladder and as he was cleaning the window, standing about 12 feet up, the ladder slipped out on the cardboard and the employee fell to the floor below. The employee fractured his elbow, hip, and leg.
