Investigation Summary
At 11:30 p.m. on February 7, 2018, an employee was standing on the water trailer fender above a wheel that was approximately 34 inches above grade. The employee was holding a water hose into the top fill opening and a coworker was at fire hydrant opening the valve. The employee lost his balance and fell from the trailer when there was a pressure surge in the hose. The employee fractured his heel and was hospitalized.
